Natalie goes into labor; a dark family secret comes to light for Dr. Rhodes; Med works to save two airplane stowaways.
After barely surviving a flight from Mexico, two airplane stowaways, who were hidden in an aircraft's landing gear, are brought into Med with critical injuries. Due to their undocumented status, Homeland Security shows up at Med to investigate them. The two are siblings, and they were hoping to escape their drug kingpin father and find a better life in America. Though Connor (Colin Donnell) is able to save the sister, the brother dies from his extensive injuries. Despite protestations from Goodwin (S. Epatha Merkerson) and Connor, Homeland Security deports the sister back to Mexico.
Dr. Choi (Brian Tee) tends to the wounds of a teenager who sustained injuries from a bicycle accident. Later, in a shocking twist, the young victim returns to Med dead on arrival from a random drive-by shooting.
Natalie (Torrey DeVitto) goes into labor. Although there are complications and she nearly loses the baby, she delivers a healthy baby boy, whom she names Owen.
Connor's father makes a substantial donation to Med, which the hospital will use to fund a new psych ward. We learn that part of the hostility between father and son is due to the fact that Connor's mother, who the father claims was mentally ill, killed herself because of her husband's philandering.
Sarah (Rachel DiPillo) faces the toughest test of her young career when one of her patients, a young boy, has an incurable disease that leaves him with a life expectancy of only five more years. Dr. Charles (Oliver Platt) helps Sarah through the difficult process of delivering the sad news to the family.
